There are two ways to which you can search for a member. We encourage using Subscriber ID as the primary way to search. However, we also allow you the ability to search by the member's name and date of birth. If more than one record is found, you will be asked to enter in additional information.

2026 Dental Coverage Updates
Effective January 1, 2026, UnitedHealthcare® Medicare Advantage plans will have the following benefit changes:
- Coinsurance added to non-preventive services for cost share plans
- D4910 no longer covered under preventive-only plans

Effective January 1, 2026, Elderplan and HomeFirst will discontinue its partnership with Healthplex as its dental network provider.
After December 31, 2025, services for members enrolled in Elderplan Plus Long-Term Care (MAP), Elderplan Medicaid Beneficiaries (SNP), Elderplan Extra Help, Elderplan Flex, and HomeFirst Managed Long-Term Care (MLTC) plans will no longer be processed through Healthplex. Consequently, Healthplex providers will be considered out-of-network for these Elderplan and HomeFirst members.
The Hawaii General Excise Tax (GET) is tax that is charged on business activity in Hawaii. The Fee Amounts listed in our Preferred Provider Organization (PPO)/Medicare and Medicaid Fee Schedules are all inclusive, including, without limitation, reimbursement for any/all applicable state and federal taxes. Utilization Management Decision Making:
Utilization management decision making is based only on appropriateness of care and service and existence of coverage. The organization does not specifically reward practitioners or other individuals for issuing denials of coverage. Financial incentives for Utilization management decision makers do not encourage decisions that result in underutilization.

At this time, you will not be able to retrieve procedure code pricing (i.e fee schedule) for these plans.
